Working with a number of stakeholders including Age UK and Public Health England, we shot a series of patient-based profile films to show how the South West London Health and Care Partnership has provided bespoke services for their patients.

We worked with the South West London Health and Care Partnership (SWLHCP) to produce a set of case-study films featuring patients who were benefiting from a unique support system coordinated by the care trust.
The films featured interviews with patients, GPs, Charity partners and Carer’s who had worked together to improve the care system around individuals and how they had gone about it to help other Trusts replicate the same models. The films were delivered to a variety of stakeholders including Age UK and Public Health England.
